Here’s what you need to know for your first PT session

We are excited to see you in the clinic! Being prepared for your first visit can make the process smoother. Read through the following, and check out the forms below to get a head start.

 

Forms

We need some information from you before your first visit. You can download and print these forms (see below), and bring them to your first appointment. Or we will have them available for you to fill out at your first visit.

Time with your PT

Your appointment is scheduled so that you have one on one time with your PT. Please come in early enough to fill out forms and take care of the financials before your appointment time. If you fill out the forms at home you will need less time to do this in the clinic!

What to bring

When you come in for your evaluation, bring the referral from your physician, your insurance card and photo ID, a list of medications you are taking, and your co-pay.

 

What to wear

We recommend wearing clothes that will be comfortable for performing exercise, and that will allow exposure of the involved area (i.e. wear shorts if you are coming in for a knee issue).

Follow up visits

Once you are evaluated by your PT, they will discuss the recommended treatment plan, including recovery time and frequency of visits.

Regular attendance and participation

In order to gain maximum benefit from PT, it is important that you participate actively in treatment, including attending appointments and performing recommended home exercises. Communication with your PT is vital to your success and will help them to tailor your program to your specific needs.
